Individualized Family
Service Plan (IFSP)

Every child who receives services from Early Childhood CARES has an Individualized Family Service Plan (IFSP).

This plan is developed with the child’s team once a year and reviewed in the middle of the year. The IFSP describes the child’s current skills and the skills the team wants the child to build in the coming year. The IFSP identifies the services the child will receive to meet these goals, who will provide the services and where they will occur.

Before Your IFSP Meeting:

  • Identify the child’s strengths and interests. What does your child like to do?
  • Decide what you want your child to learn this year.
  • Determine how you would like to be involved in your child’s learning and services.
  • Bring your notes and ideas to the meeting.
